longer be part of Empire BlueCross BlueShield’s provider networks, effective
January 1, 2017. CarePoint consists of the following New Jersey hospitals:

Hospital, 176 Palisade Ave, Jersey City, NJ. Hoboken University Medical Center,
308 Willow Ave, Hoboken, NJ. For your convenience, the following are
alternative solutions: Englewood Hospital & Medical Center, 350 Engle
Street, Englewood, NJ. Jersey City Medical Center, 355 Grand Street, Jersey
City, NJ and Valley Hospital, 223 N Van Dien Street, Ridgewood, NJ 07450.
