  At Home Over-The-Counter COVID-19 Tests for Free

Effective January 15, 2022, at home over-the counter COVID-19 tests will be covered under your prescription plan.  Please review the below question and answer guide so you can use this benefit correctly.

Q.Where can I purchase the COVID-19 Test for Free?
A.You can use your Express Scripts card at participating pharmacies. 

Q.   How many tests can I obtain?
A.You can obtain 8 tests per month for each covered individual under your plan.  For example: a family of 4 can obtain 32 tests per month free.  Any additional tests beyond your allotted quota will be at your own cost.

Q. If I do not use a participating pharmacy but purchase the tests elsewhere, how do I get reimbursed?
A.Express Scripts can provide you a reimbursement form for you to complete and submit directly to Express Scripts with the itemized receipt. You can be reimbursed up to $12.00 per test.  You can contact Express Scripts at 1-800-272-7243 to obtain the reimbursement form.
